Overview

About The Role We at Huntercombe Group are looking for a Part time Dietician to work 18 hours per week - 3 X 6 hour days. As a Dietician you will be responsible for: · Providing dietetic assessment, diagnosis and treatment programmes to individual patients. · Working as an autonomous practitioner, holding a clinical caseload and working without direct supervision. · Supporting less experienced staff and students and colleagues deliver the Company’s contracted services. Key Accountabilities: · To provide expert advice, signposting, guidance and information to health and social care professionals, patients, carers, relatives or other non-professional contacts. · To take appropriate action when working with vulnerable adults and children following guidelines and Company procedures, liaising with appropriate agencies as required. · Maintain and contribute to the standards of professional care, ethics, policies and procedures. · To accurately record all patient intervention and plans in the medical notes. · Diet related resources. · Menu planning, designing with Catering Department. · Education workshops with staff, patients, carers and other professionals. · Induction training with new staff and update as required. · Implement standardised dietetic audit. · Attend regular dietetic supervision. · Contribute to the Nutrition Steering Group. Clinical Role · To provide a high level of care to patients with dietetic and nutritional requirements. · Using highly developed knowledge & skills in the implementation and planning of individual dietary treatment plans taking into account patient’s condition. · Facilitate the safe discharge of patients with nutritional and dietetic plans. · Negotiate with carers, patients and others around individual case management. · Use specialist knowledge to inform sound clinical judgement/decision making for case management. · To contribute to multi-disciplinary clinical teams, communicating complex information to members of the teams. · Understanding the emotional needs of patients and their carers. · To maintain up to date and accurate case notes in line with professional standards and local Company Policies. · Wherever possible, include patients and/or carers in the planning of care (at home or within the hospital setting). Staff Management, training and development · To provide mentoring, advice and support to the Dietetic Support Worker and students as necessary. · To undertake training of service professionals, carers and colleagues on up to date dietetic information. · To attend relevant training and development in order to maintain the skills and knowledge required to maintain up to date HCPC and Dietetic registration. Research and Audit · To undertake clinical governance/audit projects as required. · To collect and provide research data from own caseload as required and participate on departmental research. Communication and Relationship Skills · Provide a high level of communication with internal and external stakeholders. · Participate in staff meetings offering views, ideas and opinions. · Communicate all relevant issues pertaining to care. Clinical Obligations · Responsibility to maintain continuous professional development in order to meet requirements of professional registration. · To work within the boundaries of your relevant Professional Code of Conduct. · Adherence to national guidance and best practice evidence on infection prevention and control. · Compliance with the Huntercombe Group clinical policies and those adopted. · Participation in the Huntercombe Group clinical governance work programmes.
