Qualified Social Worker - Hospital

Remedy Recruitment Group are currently seeking an experienced Qualified Social Worker to work within a Hospital Discharge Team for Wandsworth Council. Job Purpose The Richmond Response and Rehabilitation Team (RRRT) is an integrated health and social team comprising of social workers, occupational therapists, physiotherapists and nurses. The role of the socials workers within the team is facilitate safe and timely discharge. As there is no acute Hospitals within the borough the socials workers are aligned to the main hospitals that Richmond residents attend: Kingston Hospital and West Middlesex University Hospital. RRRT has strong links with both main hospitals and staff are able to work from various sites. Richmond SS has developed a Joint Assessment and Discharge Team with Kingston Hospital. Staff are rota-ed to work from Kingston within this team which focuses on supporting complex discharges and avoiding unnecessary delays. Richmond are currently placed number 1 in London in terms of DTOCs so the team has established processes. Health oversee all Reablement cases within the borough so social work staff also support those who following Reablement have been identified as having long-term needs. The Reablement work is generally managed by one staff member but if this is unmanageable due to high numbers going through the service, cases are allocated within the team.
