Job Description A permanent vacancy has arisen in the Hospital Discharge and Intermediate Care Service - Clackmannanshire & Stirling Health and Social Care Partnership. We are looking for an experienced Community Occupational Therapist (registered with the HCPC) for 35 hrs per week to support the Stirling Teams to undertake Outcome Focused Assessments and to support clients to return home from Hospital. The Hospital Team is located on Stirling Community Hospital Grounds and is made up of 2 Community Occupational Therapists, 2 Social Workers and a Coordinator. The team work to a two week assessment period, work in a pressured environment with tight timescales and with regular scrutiny from partner agencies to avoid delayed discharges. Although Hospital Discharge work will take priority there will also be a requirement to undertake assessments within Short Term Assessment units in Stirling and on occasion within the Reablement team, depending on priorities. You will be expected to work in collaboration with health colleagues and third sector partners to use Outcome Focused approaches to assess someone's long term support needs and coordinate relevant support packages. Knowledge and competence in assessing Moving and Handling needs, equipment and adaptations and managing complex discharges and risk is essential. Previous applicants need not apply. Requirements We are looking for a Community OT who is an autonomous practitioner and confident in applying core OT skills to maximise independence. You will be required to advocate for clients' outcomes despite the pressured timescales and challenges from other professionals and families. Knowledge and experience of Adults With Incapacity and Adult Support and Protection procedures is also essential in your role as a care manager.
