Darlington Borough Council is currently looking for a Social Worker to join their Adult Review team is based in Central House. The aim of the team is to undertake strengths based reassessments and reviews of individuals living in the community who are in receipt of domiciliary care services. The team consists of Social Workers and Occupational Therapists who work together to support individuals in identifying their strengths and achieving positive outcomes. Work will vary across teams and include initial contacts, safeguarding referrals, signposting, facilitating hospital discharge, support planning, s21 court report, robust assessments and reviews focusing on strengths based perspective. Carrying out a range of assessments & reviews with Adults and their carers. The successful candidate must Experience of working with clients aged 18 and above. Experience of undertaking assessments.
