Overview
Registered Bank Community Nurse, Band 5, North Bristol Community District Nursing team is looking for an enthusiastic and dedicated professional to join their team. They are privileged to offer nursing care in the home environment and work with a range of supporting services to tailor care to support the patient's needs. As a Community Nurse, you will work within a team including phlebotomists, health care assistants, assistant practitioners, nursing associates and have the supervision of a Community Nursing team leader. You will be part of the Integrated Team with rehabilitation therapist and older persons mental health team colleagues supporting patients with a wide range of health conditions. You will also be working within the new neighborhood model supporting patients to access services they need closer to home. Community nurses visit patients in their own homes between the hours between 08:00-22:00 7 days per week and have a variety of shifts available during these hours. We are looking for a nurse with a passion to deliver quality care and support. who will be able to work autonomously and as part of a team. We are interested in hearing from people who share the above values, and who feel they can make a difference for our client’s patients with their dedication and expertise. You will have excellent communication skills and be responsible for the development, implementation and evaluation of personalised care plans, to contribute to Somerset Partnership’s vision of providing excellent nursing care. Transferable clinical skills will be supported with training packages and a period of induction/orientation tailored to your needs.
