Overview

Home Carer Service users with a range of complex needs are enabled to live an independent life within their own homes through the delivery of a personal care service, including assistance with social and emotional support where appropriate to do so. Key Objectives (list what outcomes are essential) To provide a personal care service, delivered in accordance with the agreed care plan and the appropriate policies and procedures. Ensure clear, concise and accurate records are kept of each visit. Ensure changes in care plans where required, are passed to team leader for further action. Respond to changing needs of the service user. Always being aware of health and safety and following the risk assessment to ensure service users and carers safety. Other duties to be undertaken include simple dressings, stoma care and medication administration as per GP's instructions, after the appropriate training The role operates in service users homes, enabling them to live as normal a life as possible; the relationship between service user and home carer is one of trust and reliability. Communication with family, health colleagues and other agencies are vital in securing the objective of the service user remaining independent at home and not being admitted to a different care environment. There is regular contact with the line manager to ensure that changes are recognised and the service user is always receiving the appropriate care and living in a safe environment. The role holder will be expected to participate in team meetings and attend training sessions as identified through supervision and appraisal. Work Profile This is a key role demanding a high standard of care and an awareness of the special requirements associated with older people, disabled and other vulnerable groups; taking into account the diverse cultural needs is an essential element of the role. It involves mainly working alone to meet the service users assessed needs and responding to any changes or emergency situations as they arise. Individual responsibility to keep family, team leader and where necessary, other agencies, informed and up dated of any changes in situations. Once the service users needs have been professionally assessed, the role holder is responsible for delivering care in accordance with the care plan. This will include personal, health and social care. There are regular supervision sessions and at these, the role holder would be expected to input into the care plans. To ensure clear and accurate written records are kept of each visit PERSON SPECIFICATION Diploma in Level 2 in Care or equivalent Diploma in Level 3 in Health and Social Care or equivalent Requirement to undertake mandatory training to fulfil Care Quality Commission standards. Taking responsibility for delivering own work and meeting agreed work objectives. Working to agreed guidelines. Able to work alone or as part of a team. Prioritising work loads in accordance with the service user's reablement plan. Planning capacity and resources To ensure work load is completed within given timescales. Examples of activities in the role. PERSONAL CARE SERVICES to include: Washing, oral hygiene, Shaving (following risk assessment), dressing, assisting with toilet needs, bathing/showering, assisting service users to eat and drink. MEDICAL NEEDS to include: Simple dressings, stoma care, tracheotomy care, peg feed care, catheter care and medication administration as per GP's instructions and as stated on the medication chart, after the appropriate training.
