Be a Case Worker This role is all about supporting participants through barrier identification and resolution, to enable them to move closer to engaging with mainstream provision and/or undertake training/move into sustainable employment. In brief you will: Source suitable clients onto provision via means of liaison with CRC, NPS, Prisons, Police and other key stakeholders and to coordinate an effective through the gate service. Undertake thorough assessments to identify barriers and implement appropriate support plans to meet the needs of programme participants. Manage a caseload of clients on a one to one and group basis. Utilise a variety of means to facilitate improved social inclusion. Identify and coordinate a range of specialist support interventions utilising internal and external partners to effectively address barriers.
