Job Title: Community Care Officer -East and North Herts Older People's Service, Integrated Hospital Discharge Service Location: Princess Alexandra Hospital - Harlow Salary Range: £22,462 progressing to £29,636pa Hours: 37 hours per week Contract: Permanent Job Duties: Hertfordshire County Council are looking for a Community care officer. You will be responsibility for assessing and arranging care in a hospital setting. We are a busy team covering a large and diverse area and would welcome applications from candidates who can bring energy, creative care planning and a citizen focus. Staff will be asked to work on consecutive Saturdays and Sundays once every eight weeks and will receive days off during the weeks before and after covering weekends.
