Overview

Job Summary The main purpose of this role is to manage rosters for carers which are fair, consistent and realistic for carers and clients. The role also involves the mentoring, monitoring and coaching of Carers, reporting concerns regarding Service Users and Carers to the Line Manager. This role is performed in accordance with the Employee Handbook and Healthvision’s policies and procedures. Key Accountabilities Rostering of Carers Assigning appropriate carers Follow rostering guidelines, which include consistency of carer and appropriate travel time Rostering of care staff for training at the request of the Care Supervisor or Registered Manager Client Relationships Any change of carer/ time should be communicated to the client Regular feedback through telephone conversations to ensure service satisfaction Electronic Monitoring Ensuring all staff log in and out correctly Following up on any anomalies Providing accurate details for payroll and billing Managing Care Team Consistent support and regular feedback for carers in all aspects of their role Effective communication with carers Build rapport and strengthen the professional relationship with carers Show understanding and empathy towards carers Reporting Mechanism There is the expectation that the Care Coordinator will communicate regularly with the Care Supervisor and report to the Registered Manager. Any serious or extremely serious care or client health concerns should be reported immediately to the Registered Manager and external agencies as appropriate. Business issues, such as budgets and invoicing queries should be communicated to the Business Manager or the Managing Director. Serious disciplinary issues need to be addressed with the Registered Manager and HR. Personalization Healthvision actively promotes the wishes and choices of Service Users to ensure they receive a personalized service. As a Care Coordinator, you will need to be responsive and flexible to changing needs of our Clients where practicable and within Healthvision’s policies and procedures. Outcome Focused Health Vision follows the ‘outcomes’ model of care, where specific goals and desired outcomes are planned, with the input from the Client, at the beginning of a care package. Delivery of care is always undertaken with these outcomes in mind, with regular feedback from carers essential in monitoring the progress of the Client. Working Relationships Most common external contacts: Clients and their family Community Care providers, and Other Health professionals Social Workers Brokerage Teams Most common internal contacts: Care Team Registered Manager Care Supervisors Human Resources Position Requirements Preferably with a minimum of 2 years’ experience in a busy office environment. Someone who is an excellent communicator and uses their own initiative Experience in the “Care Sector” would be an advantage People management skills Organizational skills Can work well under pressure, is hardworking, a team player and a real go-getter. Should have obtained an NVQ L3 in Health and Social Care (Desirable but not essential). Ability to establish relationships under challenging circumstances Good written and oral communication skills Commitment to being sensitive to the needs of other cultures Proficient use of IT systems Quality Health Vision has a commitment to continuous improvement Healthvision will initiate an annual performance review Any problems or issues will be communicated to the Registered Manager Healthvision will organize training sessions where needed.
