JOB ROLE : Support Worker JOB TYPE: Permanent HOURS : 36 hours per week, Various shifts: 8:00am - 2:00pm / 11:00am - 6:00pm / 1:00pm - 7:00pm / 2:00pm - 8:00pm. Purpose of job To deliver a person-centred integrated support service for participants in the Community Living Service, and to discharge responsibilities within an allocated registered service. Main accountabilities To provide personal, intellectual, social, emotional and physical support to participants. To follow agreed standards and procedures as determined by CQC, the company and other agencies. To support the participants in accessing both community and leisure activities, and working with them in order to facilitate experiential learning opportunities. To support individuals to achieve their goals and aspirations through person centred planning. To provide support to participants in maintaining home management skills and finance enabling them to ensure the security of their placement / tenancy. To carry out prior risk assessments of community venues / activities and participants need, in order to ensure the safety of the individuals participating. To work alongside individual participants enabling them to develop their own individual skills and abilities that will allow them to achieve greater independence and autonomy. To act as a co-ordinator for certain participants acting as advocate where appropriate. To be responsible for the safety and welfare of individuals who have multi-disabilities, and to ensure their safe participation in the project and the community To act as a representative of the service in all community dealings and actively promote the awareness of rights, respect and responsibilities for disabled members of the community. To assist in the writing of reports and attend reviews where appropriate and maintain records of individual progress and achievement, and support participants in maintaining their own records and personal portfolios. Person specification All criteria are essential unless otherwise stated. Specialist knowledge, skills and experience Understanding the needs of blind and partially sighted people with additional disabilities and a passion for caring for people Willingness to complete the company induction programme and our care certificate. Good level of literacy and numeracy and computer literate with a good knowledge of Word, email and the internet. Ability to use a word processor, with knowledge of the internet and email. Willingness to develop skills in this area. Desirable Experience of working in a social care or residential environment to include facilitating development of personal care skills and the preparation of meals. Team working Ability to develop effective and supportive relationships with colleagues, participants, parents and carers. Special conditions Appointment is subject to an Enhanced Disclosure and Barring Services Check for regulated activity Shift and overnight work required to cover service requirements across 24 hours a day, 7 days a week and 365 days a year.
