Purpose of the role As a Nurse Care Manager your role is to provide effective leadership and clinical management of our care packages. Ensuring the professional delivery of high quality care within a defined group of service users to support them to live safely in their own homes and achieve the lifestyle and outcomes they want for themselves. You will complete regular service user reviews and manage the competencies of carers as well as liaising with other care professionals and providing feedback and reports to the Registered Manager. The role is 40 hours a week but requires the post holder to be flexible to ensure the care services are safely managed. Reason to Join Sugarman Health & Wellbeing: £35,000-£37,000 salary £3,000 car allowance per annum 25 Days Annual Leave per annum Paid travel expenses NMC Revalidation fee paid for by the company On going training and development Access to a pension scheme Full Enhanced DBS paid for by the company Access to Refer a Friend scheme where you could receive £500 Support network of highly skilled Directors, Managers and Nurses Main Responsibilities: To deliver productivity and quality standards agreed between yourself and the management team To undertake new assessments when required and plan with the team to provide care for new care packages To work unsupervised and use own initiative however know limitations and ask for support where necessary Work well as part of a multidisciplinary team and build positive working relationships Complete clinical competency sign offs with carers Complete care needs assessments, risk assessments and reviews Apply professional skills and manage own professional competence and accountability, in accordance with the NMC Code of Conduct Clinical Care Delivery: Provide continued clinical input and support for staff in the daily delivery and support for individuals in their own home, ensuring that each person supported has their individual clinical care plans implemented and evaluated as required, in order to accommodate their changing needs. Effectively delegate and supervise care staff in order to ensure all identified clinical and care needs are met. Have a clear understanding of personal accountability, undertake clinical procedures and training with individuals in accordance with care plans and sign off required competencies in a timely and safe manner. Providing verbal and written reports/review on service issues and individual's progress to the Registered Manager. Report real time incidents, accidents or complaints to the Manager in charge, in line with company policy and process and make necessary regulatory notifications in consultation with management. Record, audit and complete any appropriate documentation and records/systems in order to ensure that individual's records are accurate and informative. Communicate and coordinate with other Health and Social Care Professionals and Medical Practitioners, to ensure a comprehensive and cohesive clinical approach in all planned actions and interventions for the health needs of the individual's you support.
