Overview

A charitable organisation who have been established for over thirty years in North London and provide (along with many other services) tailored home care services are seeking to add an experienced care-co-ordinator to their established team in order to successfully ensure that all home care clients needs are met by recruiting, booking and ensuring that quality care is delivered at all times. The role of the Home Care Service Coordinator is to ensure the continuity of care while actively promoting the services that the charity offer, through face to face and telephone communication. Ensuring that services have a sufficient recruitment pipeline to meet enquiries and developing opportunities with potential clients to increase the profile of the service across the territory. Additionally, maintain a good working relationship with Service users and family members is paramount. Salary of Care Coordinator - £23,000 Responsibilities of Care Coordinator: Manage the activities of a workforce ensuring that the needs of service users or clients with each of their designated areas are met. Support in the day to day activities within the Home Care teams or as the needs of the service dictates. Ensuring that service users lives are enhanced by the strength of a well managed, reliable & stable care team. Providing a crucial link between the service user, the care teams and any other parties involved. Understanding and working within the structure and ethos of the company brand, with its mission, vision and values. Organising Care Worker rotas to minimise changes to the service and travel between service users homes. Responding efficiently day to day changes in the care and support packages. Providing returns and reports at agreed intervals, detailing service activity and time Produce management information on a regular basis detailing levels of enquiries, conversation ratios and service development Adhoc management information and reporting as required by offices and management. Handling incoming Home Care enquiries in conjunction with the relevant teams via telephone, online enquiries and face to face recommendations of referrals. Qualification and follow up on current and historical enquiries to ensure a maximum number of conversations from the enquiry. Be able to give formal and informal presentations to service users / Local Authorities Ensure that all service users’ needs are assessed and met in a professional manner. Follow all company quality procedures and carry out internal audits both formal and informal. Investigate and resolve minor complaints and ensure more serious complaints are referred. Ensure that the quality of each care worker is regular assessed and escalate to Managers when workers do not meet acceptable compliance levels. Ensure that regular telephone contact is made with clients and logged appropriately. Hold regular service improvement meetings with major clients- keep records. Participate in training sessions agreed with your line manager. Maintain the highest possible personal and professional standards in terms of personal presentation, method of operation and ethical judgements. Essentials of a Care Coordinator Excellent planning and IT skills Ability to establish and maintain effective and strong working relationships A natural ability to empathise and the ability to deal sensitively and compassionately with service users and staff needs. Ability to work with teams to help to ensure deliver high standards of care. Ensure that service users are protected, and that dignity is always maintained. Ability to remain calm, positive and to work under pressure with professionalism,motivate and enthusiasm. Full flexibility to meet the demands of the service. Experience in marketing a service Must be committed to anti-discriminatory practise and service user rights
