We are looking to recruit a Care Team Leader for a new service starting in the Oldham area of Greater Manchester Job Purpose As a Care Team Leader you will support all kinds of individuals with different needs, in different ways including: People who have a learning disability and/or; Complex health needs Autism and Autistic Spectrum Disorder Profound and multiple disabilities Asperger’s syndrome Mental health problems Physical disabilities Communication difficulties Complex behaviour Sensory impairments Job Description As a Care Team Leader, your duties will include but are not limited to: Support people with regards to decision making and lifestyle choices enabling them to take control of their own lives, increasing independence. Ensure all personal care tasks are carried out in line with the needs, wishes and preferences of the individual, following the care plan. Support people to take an active role in the community by accessing local facilities and wider community resources in the spirit of social inclusion. To provide rapid access to competent advice and expertise to both support workers and people who use our services. To provide effective support, training and supervision to a team of Support Workers. To support management of risks and care/support delivery. To support the establishment and stabilization of new support packages To source / provide cover for support workers during periods of absence. To provide direct support to people who use our service, particularly with new or complex cases or when staff cover is required. To help service users maximize their independence, reduce their dependence and to encourage “joint working” by Support Workers with people who use our services wherever possible. Day to day responsibility and monitoring of the work of a team of Support Workers and working alongside specialist teams as appropriate. The overall responsibility for all support provided in your service. Assist the Service Managers with the planning of the rota/s in your area on a weekly basis, utilizing a computerized system. Supporting new support staff in the shadowing process at commencement of employment with Liberty Support. Represent your area at all monthly Planning/Team Leader meetings. Informing senior management of any care/support, disciplinary or other relevant issues as soon as practically possible. To assist and operate the on-call duty system on a rotational basis with other senior members of staff. Liaising with service users and their families Becoming a Care Team Leader requires you to work flexibly, this will include working evenings, weekend and sleep in/waking night duties as required. Liberty Support offer a continuous service, 365 days per year.
