Overview
Facilitation Officer Location: North Central London Contract: Fixed Term contract Work Pattern: 4 Days Per Week ICS Health & Wellbeing is a dynamic and accomplished organisation dedicated to making the UK’s local and workplace communities healthier. Working with local authorities, CCGs and employers, we design, implement and evaluate innovative health and wellbeing services that will result in long-term health benefits through sustained behaviour change.We have been selected to deliver the NHS Diabetes Prevention Program (NHS DPP) across the UK. NHS England, Public Health England and Diabetes UK have committed to improving efforts to prevent diabetes, and this collaborative program aims to help the 5 million people who are at risk of developing the disease. We are currently recruiting for a Facilitation Officer on a 12-month fixed term contract to help support the coordination between the GP practices North Central London. Duties: Support GP Practices: Provide support to the delivery of the National Diabetes Prevention Programme across North Central London region, specifically focussing on engagement with GP practices identified as priority by Public Health and the CCGs. Referral Generation: Coordinate with GP practices to help increase GP referrals for ICS Health & Wellbeing. Visiting GP practices across North Central London to support raising awareness of the NDPP programme for ICS Health & Wellbeing. Collaborate with the Service delivery and primary care teams to better support new strategies to increase GP referrals. General Support: Develop and distribute NDPP comms with primary care and other partners to ensure smooth communication and coordination. Establish and maintain administrative systems to support the delivery of the NDPP such as information management systems and communication functions. Collaborate with project members to help support the collection of information and produce reports on the NDPP to NHS England. Act as the main point of admin contact for GP practices to better support complex queries from Stakeholders. Experience and Qualifications: Previous Facilitation or Coordination experience within a Healthcare capacity is essential. Experience of dealing with sensitive/confidential information. Previous experience of coordinating administrative tasks/procedures. Proven experience of demonstrating strong relationship building skills across Stakeholder channels. Competent user of IT or Hospital electronic systems is desirable.
