Your new role This is an exciting opportunity for a dynamic Social Worker to work in the East Locality Team on a part time permanent basis. The team are undergoing a large transformation programme across adult social care to develop person-centred pathways and practice which is enabling, supportive and developing communities to become more resilient. Working alongside health colleagues to prevent adults from being admitted to long term residential care and supporting interdependence with the community, you will support adults and their carer's as a 'change agent' to build on their strengths, assets and abilities to self-manage. This post not only supports the statutory requirement for a Social Worker to undertake timely reviews, it also promotes an asset-based approach, maximising independence of the adult and reducing marginalisation. The position will ensure that through a person-centred approach that adults are assessed in a timely and appropriate manner to prevent adults from being admitted to long term residential care and supporting interdependence with the community and to return home as soon as possible, to ensure adults do not remain in placement any longer than is required which can have detrimental impact on their physical, emotional and mental wellbeing. This is a 26 hours per week position, the manager is happy to look at other hours as well What you'll need to succeed To be successful in this position it is essential that you have a qualification in Social Work (Diploma/Bachelor/Master's degree) and hold a current registration with Social Work for England. It is essential that you possess a good working knowledge of the Care Act 2014 and Mental Capacity Act 2005. The post holder will need to have a sound knowledge of the SET Procedures for Safeguarding Vulnerable Adults and assessment and investigation skills. You will need to demonstrate that you have a proven ability in ensuring that adults and older people are appropriately safeguarded. It is desirable that you have some hospital and/or community experience to understand the interface and delivery of care between Health and Social Care, to include a working knowledge of Continuing Healthcare.
