Overview
We are looking an experienced Social Worker to cover a vacancy within the Physical Disabilities team. This is 3 days per week post. We are looking for a candidate with experience of undertaking strength based assessments, outcome focused approach and ability to coproduce a spectrum of innovative and creative solutions. The successful candidate will need to have a sound understanding of the Social Services and Wellbeing Act (Wales) 2014, Mental Capacity Act 2005, Multi-disciplinary working; NHS Continuing Health Care (CHC) and the ability to manage a complex case load. Effective communication skills including verbal and written, are key to this role as is having the ability to create and maintain good working relationships with all parties. As this post will result in substantial contact with adults at risk/or vulnerable adults the successful applicant will be subject to a Disclosure and Barring Services check. The successful candidate will also need to register with Social Care Wales if not already holding registration.
