Overview

Synopsis: Tradewind Social Care is looking to recruit an Adult Social Worker to join a Community Mental Health Team in London The main purpose of the role is to work as a part of a multi-disciplinary team to provide responsive and reactive high quality community care services, which will enable people to lead as independent life as possible, and have meaningful choices. Main Duties: To carry a caseload of people with severe and enduring mental illness and establish pro-active intervention in service user centred ways To carry the responsibility for the development, implementation and evaluation of care for clients with severe and enduring mental health problems To undertake carers' assessments in line with the Carers recognition Act To be a member of a mutli-disciplinary team and work in close cooperation with hospital based and primary health care based professionals, the voluntary and independent sectors, services users and carers Undertaking assessments of risk and formulating care plans to manage and minimise risk Planning, implementing and reviewing of ongoing interventions designed to meet assessed need To participate in Team Meetings, initiating and discussing new methods/models of care and participate in the introduction of research or trials within the area To ensure that an integrated service is provided to clients and ensure that a good inter-disciplinary working environment is maintained Essential skills & experience needed: Experience of working in the mental health field Have a knowledge/understanding of the legislation likely to be encountered as part of the role e.g. Mental Capacity Act 2007, Mental Health Act 1983, Community Care Act 1990;Misuse of Drugs Act 1971 Access to Information Act 1986; Health and Safety at Work Act 1974 A strong commitment to services for adults with mental health problems and in improving service delivery to meet the needs of service users and carers Job Requirements: An enhanced DBS that is of satisfactory level is essential Must have a minimum of 2 years experience in a similar role Must be Social Work qualified with a valid SWE registration This job was originally posted as www.totaljobs.com/job/89933258
