Overview
Morgan Hunt are currently working with an organisation to provide support to vulnerable individual including those with Mental Health needs, substance misuse problems, homeless families, asylum seekers and high risk complex backgrounds. Duties involve: A combination of housing related and support related packages aimed at preparing individuals to take on their own tenancy Providing person centred care plans which allow for a strong and trusting relationship to be developed with the Support Worker Encouraging and developing daily living skills including cooking, budgeting, and attending medical appointments 1:1 support packages offered to those who are high risk and the most vulnerable individuals Keeping accurate up to date records around the progress of individuals Multi-agency working and liaising with external organisations that can also offer support tailored to individual needs In order to be suitable for this vacancy, you must have experience in working with complex client groups and be confident in lone working. You must be flexible in your availability to work and hold a current Enhanced DBS on the update service.
