Overview

Support Worker The role of Mental Health Support Worker involves managing a caseload of clients within our Care Unit and Supported Accommodation. You will be involved in assessing prospective new clients, booking new clients in and completing Induction Support Plans, as well as creating and implementing full Support Plans and Risk Management Plans for your own caseload. You will regularly liaise with Health Professionals and other agencies to ensure delivery of a holistic support service. You will also be involved in the delivery of activities. Mental Health Support Worker Requirements: You will have a keenness to make a positive contribution to improving the quality of life for people who have a range of needs, whilst being highly motivated and proactive with a determination to succeed. You will have a working knowledge of the needs and support requirements of people with complex mental health needs, as well as a proven experience of preparing and implementing needs assessments, support plans, risk assessments and management plans as well as individual action plans in conjunction with other workers / agencies and clients. Proven experience of working with and managing a caseload of clients who have a mental health illness or within another social care setting is essential.
