Overview
Adult Social Worker Norfolk Community Care Services 3 Month Ongoing £30ph I'm recruiting for a Qualified Social Worker to be part of the community care team and take on cases of adult and older people on the current holding list. Suitable candidates will be able to work from such places as Norwich, Kings Lynn & Great Yarmouth. Duties: 1. To manage a caseload as allocated. 2. To undertake assessments under the Community Care Act and other legislation, using a self directed support approach. 3. To co-ordinate and monitor care packages and support (care) plans and undertake reviews to ascertain whether the desired outcomes are being achieved. This may include advocating on behalf of service users or carers. 4. To work with individuals to assist them to assess risk and take decisions related to their ability to function in their community environment. 5. To lead Safeguarding Adults investigations using the local 'No Secrets' multi-agency policy and procedure 6. To promote safeguarding awareness with service users and multi-disciplinary colleagues. Keep updated on current safeguarding guidance and practice and be vigilant in all activities. 7. To take a lead in complex cases such as those with safeguarding issues, complex family dynamics, conflict and problematic situations. Please note, a job description can be provided upon request. Requirements: - Social Work Qualification - HCPC Registration - Prior experience within a qualified adults role - Access to a vehicle and clean driving licence.
