Health Visitor

Health Visitor Hays Healthcare are currently working with a leading private organisation to recruit an experienced Health Visitor to join them on a full/part-time basis across South Essex. Health visitors carry continuing responsibility for the assessment of health needs together with the development, implementation and evaluation of packages of care. The team in Essex is an innovative new service, being delivered in partnership with Barnardo's, which delivers holistic health care for children and families from pre-birth to the age of 8. Responsibilities of the role includes; Plans, develops, implements and evaluates programmes and works effectively with a wide range of diverse people to identify their concerns about health and well-being and the target groups for any programmes using appropriate tools (e.g. Family Health Needs Assessment Tools; Common Assessment Framework). Responsible for referring children and young people to other disciplines and outside agencies. Responsible for working with the multi-professional team to facilitate multi-agency and interdisciplinary working. To promote and maintain an effective service to clients, using the assessment framework, communication, negotiation and collaboration. Responsible for undertaking health promotion activities with children, young people and their families. To work within the Children Act 2004 and act as an advocate for the voice of the child. Assesses, plans, implements and evaluates packages of care. In order to be considered for this role; you must have a valid NMC registration alongside recent experience as an RGN / Midwife.
