Health Navigator (Community Support Worker) - Portsmouth Fixed Term - 9 months Part time, Wednesday to Friday - Start and finish times negotiable £9,656 progressing to £10,728 Are you looking for a challenging and rewarding role where you are able to empower people every day to move towards independence? You will be working as part of a multi-disciplinary healthcare team, funded by Public Health England to provide support to our homeless clients and ensure they have access to health provision, located on the site of Portsmouth Homeless Day Service, Hope House hostel and community settings. You will love helping others and enjoy supporting individuals to achieve their goals and to become more independent. Your positive attitude towards your work and others will help to contribute to a great working environment. You will have excellent interpersonal skills, a calm and personable manner and be able to handle potentially difficult situations. This position is part time, 17.5 hours, working between the hours of 8am - 8pm. Day in the life of a Health Navigator : You will have a proven ability of delivering support to diverse clients with complex needs You will have a proven ability to work effectively within a multi-agency context and report to the team lead (modern matron) You will be able to advocate for clients by developing positive relationships with other agencies and be the single point of contact You will build on these positive relationships with agencies to identify and reduce the barriers within existing systems to allow clients improved access to health services You will be required to work using your own initiative You will be required to work alone with clients Support clients to appointments where required Engage with and support clients to improve their health needs by supporting them to engage with and sustain contact with the healthcare team Engage with and support clients to access specialist and relevant secondary healthcare services to support their mental health, physical health and substance misuse needs Support clients to achieve positive health outcomes in order to reduce impact on emergency services and the local community Follow organisational and statutory safeguarding procedures Maintain accurate records of all clients who you come into contact with to enable concise data collection Attend relevant community meetings/forums to ensure best practice Identify opportunities for service development Develop and maintain excellent relations with the local community Any other reasonable task as requested by a member of staff About us We are a homeless housing association, offering supported accommodation in Hampshire, Berkshire and West Sussex for those who are homeless, vulnerable or in need of support.
