Our Practice Nurse role involves: Maintain patients physical health, offering advice and support for their continued well being Salary £30,906 pro-rata Hours of work: 30 hours per week To be successful as Practice Nurse, you’ll need Take the lead in developing best practice and provide clinical advice to ward based staff on patients physical health related issues. Establish and implement an annual physical and full medical screening for all patients, advising on well men issues. Establish and implement a screening process, taking bloods where necessary, to meet the mandatory testing requirements for prescribed medication. Provide a programme of health promotion activity for patients to include well men’s issues, healthy lifestyles and smoking cessation. Assess and manage minor physical ailments referring to other professions as required. Liaise and co-ordinate our visiting GP service. To work collaboratively with specialist service providers i.e. physiotherapy, dieticians, and chiropodist. Supervision of junior staff. Personal development needs. Where you will be working Location : Victoria Road, Huyton, Liverpool, L36 5SA You will be working at V ictoria Gardens, a specialist locked rehabilitation for people with complex mental health needs and behaviours that challenge, as well as a ward for older females. You will support the service to equip service users with the skills required for community living and increased independence. The service is a recovery-based service and incorporates elements of the Good Lives Model, supported by individualised interventions. You will work as part of the multidisciplinary team which includes Nursing, Consultant Psychiatrist, Psychologist, Occupational Therapist and Social Worker support.
